RESOURCES:
Conveniently located in the menu bars to the left are all risk management related forms and resources. The forms are identified in two catagories: "Electronic-Submit" and "Print-Submit".
CLAIM GUIDLINES:
The following guidelines will help you in determining the Loss Notice/Claim Form necessary for your particular situation.
1) If the property that was damaged is an auto owned by CCC, an auto borrowed, leased or rented by CCC, or if a CCC auto hits another vehicle, then the form to complete is the AUTO LOSS [ex: borrowed van (or rental from Enterprise-Rent-A-Car) is damaged in accident]. You would also complete this form for any injured passengers or bystanders involved in an auto accident.
2) If the property that was damaged is not owned by CCC but rather by a third party and it was not in the care, custody or control of CCC at the time of the incident, then the form to complete is the GENERAL LIABILITY LOSS (ex: lawn mower throws rock and rock hits windshield of passing vehicle).
3) If a person is injured while participating in a sponsored activity, then the form to complete is the MEDICAL PAYMENTS CLAIM (ex: church member or visitor falls down stairs; pathfinder injured during camporee; or volunteer worker during clean-up day falls from ladder).
4) If the property that was damaged is owned by CCC and/or in the care, custody or control of CCC and it is NOT an auto, then the form to complete is the PROPERTY LOSS (ex: church building is vandalized or church member's piano while on loan to church is damaged by water).
